Medical Conditions A–Z:
Coding & Documentation Guide
ICD-10 coding guidance for 9,000+ medical conditions. Coding risks, documentation tips, and FAQs for every condition — from someone who has coded them all.
The Short Version
The Medical Conditions database is an A-to-Z reference for every diagnosis you are likely to code. Browse alphabetically or search by name — each condition has its own page with the ICD-10 codes that apply, the coding risks and audit flags to watch for, condition-specific FAQs, and documentation tips that tell you exactly what the clinical note needs to include.
It is not a substitute for the ICD-10-CM code book or the official coding guidelines. It is a practical, real-world companion built from thirty years of experience — the things I wish someone had told me about each condition before I coded it for the first time.

How to Use the Conditions Database
Browse Alphabetically or Search
The conditions database is organized alphabetically like a medical dictionary. Browse by letter — A through Z — or use the site-wide search to find a specific condition by name, keyword, or related ICD-10 code.
View Condition Details
Each condition page shows a full description, all related ICD-10-CM codes, coding risks and audit flags, condition-specific FAQs, and documentation tips. All the information you need to code that condition correctly, in one place.
Review Coding Risks & Documentation Tips
The coding risks section flags common pitfalls — specificity requirements, laterality issues, Excludes1/Excludes2 conflicts, and documentation gaps that auditors look for. The documentation tips section gives you the specific language to include in the clinical note to support the code.
What Each Condition Entry Includes
ICD-10 Codes
Every condition page lists all related ICD-10-CM codes — the billable codes that are directly linked to that condition, plus related codes for associated manifestations, complications, and alternative diagnoses.
Coding Risks
Each condition includes a coding risk assessment that flags the most common errors coders make. Things like: 'This condition requires a 7th character — do not forget it,' or 'This code has an Excludes1 conflict with a commonly co-diagnosed condition.'
Condition-Specific FAQs
Frequently asked questions tailored to each condition. These are the real questions I have heard from coders over thirty years — 'Can I code this with a history of the same condition?' 'Does this code require a specific lab value to be documented?'
Documentation Tips
Specific, actionable guidance on what the clinical documentation needs to include to support each code. These tips come from thirty years of reading audit findings and appealing denials.
Tips from the Trenches
Three things I have learned about using a conditions reference effectively.
Always Check the Coding Risks Before Submitting
The coding risks section is the most valuable part of each condition page. It tells you exactly what auditors look for with that specific diagnosis. I make it a habit to check the coding risks before I submit any claim with a condition I do not code every day. It takes thirty seconds and it has saved me from countless audit flags.
Use the Documentation Tips to Write Better Notes
The documentation tips are not just for coders — they are for providers too. I have worked with practices that print the documentation tips for their top 20 conditions and keep them at each provider's workstation. When a provider documents a specific condition, they glance at the tip sheet to make sure they are including the key elements. The result is cleaner charts, fewer queries, and denials that are easier to appeal.
Conditions Change — Check Back After ICD-10 Updates
ICD-10-CM is updated every year on October 1. New codes are added, existing codes are revised, and sometimes a condition that was coded one way for years gets a completely new code. The conditions database is updated to reflect the latest code set, but if you code a condition that had a recent code change, check the condition page after the update to see if anything has changed.
